On the morning of Novemeber 1, 1893, the Inez slid from the rails and fell through an open draw on the Madison Street Bridge into the Willamette. Seven died in what Richard Thompson writes was "the worst accident in Portland streetcar history" (Thompson, Images of Rail: Portland Streetcars, 2006, 39).
