I have verified the residential address that you provided lies within the limits of the City of Portland. Multnomah County Elections Division has verified that you are an active voter within the City of Portland as of 1/6/2012. You have met the requirements to file for the office of City of Portland Commissioner, Position No. 4 in the 2012 Primary Election and your name will be included on the ballot.

If you would like to be included in the Voter’s Pamphlet, you will need to contact each county’s election division (Washington, Clackamas and Multnomah), and complete all necessary forms and pay the required fees by March 19th, 2012. Links to these forms are available on the City Election website.

Your name will appear on the ballot unless you submit form SEL 150, Withdrawal of Candidacy or Nomination. You must submit this form by March 9th, 2012 in order to withdraw your name from the ballot.
