UASI Grant Development
This Contract was made and entered into on the __24th___day of __December_, _2003, by and between CDA Consulting Group, Inc., hereinafter called Contractor, and the City of Portland, a municipal corporation of the State of Oregon, by and through its duly authorized representatives, hereinafter called City.
1. The City and Contractor agreed that additional hours were needed to complete the scope of services through the end of the contract period June 30, 2008 previously assigned in Amendment 4 for the UASI FY 2006 grant period. Contractor has performed for that work in the amount of $28,572.82. However, Contractor has not been paid for that effort due to the delay in receipt of the grant from the State of Oregon. The City accepted the grant, Ordinance # 181577 on January 30, 2008. The UASI FY 2007 Grant period is November 1, 2007 through May 31, 2010.
2. The City needs the Contractor to perform additional Scope of Work to include the Urban Area Security Initiative (UASI) for FY 2007 and FY 2008 Grant Application process as follows: (See Exhibit A – Project Information and Scope of Work). The amount of the new, additional work will not exceed the sum of $258,775.

3. Deliverables — all deliverables must be provided to project manager via electronic mail. A bi-weekly scope of work or projects outlining items being worked on shall be submitted to the project manager bi-weekly. Project manager will inform consultant within five (5) business days if deliverable is acceptable. Upon acceptance of deliverable, Consultant will invoice City.
4. The total cost of work to be performed under the new scope of work shall not exceed $230,202.
5. Contractor shall perform the new scope of work for the sum of $230,202 based on an estimated 2436 hours.
6. The City reserves the right to increase or decrease the demand of services within the scope of work at any time during the contract by modifying or eliminating any task or deliverable and to reduce the total value of the contract in accordance with the Contractor pricing assumptions included in the Statement of Work; Cost Table. The City shall provide Contractor with at least five (5) days written notice of its intent prior to a decrease in the scope of work and amendment to the total value of the contract.
7. Additional compensation is necessary and shall not exceed $258,775 and is subject to 100 percent funding through the UASI FY07 grant.
8. Compensation for entire contract #35123, including amendment #05 shall not exceed $ 1,013,905.
9. The effective date of contract #35123 shall be extended to December 24, 2008.
10. All other terms and conditions shall remain unchanged and in full force and effect.
