AMENDMENT NO. 1

Amend Agreement No. 34907 between the City of Portland and Northwest Pilot Project to assist homeless and at risk seniors to obtain and maintain permanent affordable rental housing. The reason for the amendment is to increase the contract by $30,000 in Pathways funds. This will bring the total contract to $457,458.

Amend Section II. Performance Measures to add 20 additional homeless individuals and couples to be placed in transitional housing. Once completing their transitional housing at least 80% of households who are placed into permanent housing or prevented from eviction are to achieve a minimum of one-year stability. At least 70% of households who are placed in transitional housing are to achieve a minimum of one-year stability.

All other terms and conditions of Agreement No. 34907 between the City of Portland and Northwest Pilot Project shall remain the same.
