AMENDMENT NO. 3 TO CONTRACT 33011

This is an agreement that extends the agreement between the City of Portland and Providence Health System, for provision of services to the City.

1.  The parties agree that their agreement shall be extended through June 30, 2003, during which Providence Health System shall continue to provide the services specified in Contract No. 33011

All other terms and conditions of Contract No. 33011 shall remain in effect.
