ORDINANCE No. 177016
* Amend Agreement with Oregon Medical Laboratories to provide occupational health lab services and provide for payment. (Ordinance; amend Contract No. 33505)
The City of Portland ordains:
Section 1. The Council finds:
1. The City entered into Contract No. 33505 on April 16, 2001, with Oregon Medical Laboratories (OML) to provide occupational health lab services for Portland Fire & Rescue (PF&R).
2. The original contract was not to exceed $20,000 and expired on June 30, 2002.
3. OML is willing to maintain the same price agreement for one year, with a not to exceed $10,000 amendment, making the total cost of the contract not to exceed $30,000.
4. PF&R would like to extend the existing contract until June 30, 2003.
5. Funding for this service is included in PF&Rs FY 02-03 budget.
6. Amendment No. 1 to Contract # 33505 is attached, signed by the contractor and approved as to form by the City Attorney.
NOW, THEREFORE, the Council directs:
a. The Commissioner in Charge and Auditor are authorized to execute an amendment to Contract No. 33505 with Oregon Medical Laboratories, attached hereto as Exhibit A, at a cost not to exceed $10,000.
b. The Commissioner in Charge and the Auditor are authorized to draw and deliver checks chargeable to the FY 2002-03 Budget, Account 521000, Center Code 12430250, when demand is presented and approved by proper authorities.
Section 2. The Council declares that an emergency exists because the contract has already expired; therefore, this ordinance shall be in force and effect from and after its passage by the Council.
