AMENDMENT #3 TO CONTRACT

CASCADE OCCUPATIONAL MEDICINE PHYSICIANS, INC.

The current amendment to this contract expires on June 30, 2002. Per Contract #33222, this contract may be extended for additional periods, at the sole discretion of the City. At this time, it is desirable to extend this contract for one year with an increase in compensation to the Contractor of $100,000 from July 1, 2002 through June 30, 2003 for a total contract value of $275,080.00. The costs for the services outlined in Exhibit A (ii) shall be amended and adhere to the following schedule:

1014  DPSST Physical Examination  $ 98.00

Police Corps Physical Examination  $ 54.00

1010  Stress Treadmill  $175.00

1009  Audiogram  $ 16.00

1425  Contract D/S Coll. Non Nid  $ 12.00

1633  Hep. C Antibody  $ 36.00

1850  Lab-Blood Draw, General  $ 10.00

1884  Lab-Multi Chem w/CBC Mani  $ 27.00

Based upon this schedule the Police Bureau’s Personnel Division shall be billed $374.00 for community police officer applicant examinations. The Police Corps shall be billed $245.00 for Police Corps prospective participant examinations.

The City’s Project Manager for this contract is Captain Lynnae Berg.

Effective Date and Duration

This contract shall become effective on the date at which every party has signed this contract. This contract shall expire, unless otherwise terminated or extended, on June 30, 2003. This contract may be extended for additional periods, at the sole discretion of the City for additional periods. The total contract period shall not exceed five (5) years.

All other conditions of this contract remain unchanged.
